Because we are not Medicaid-enrolled, Medicaid will not cover prescriptions prescribed by our clinicians. As such, we are unable to accept patients who have Medicaid as their primary/only insurance.
The 60-minute in-person, initial appointment with Dr. Naser or with one of our certified nurse practitioners, Ms. Terri Leone and Ms. Maria Capriotti, will focus on your medical history, current symptomatology, potential diagnosis and care plan.
The 90-minute, in-person initial appointment, with Dr. Naser or one of our certified nurse practitioners will focus on the child's medical history, current symptoms, potential diagnosis and care plan. We request that both parents are present for this initial appointment.
Our practice policy requires that each parent signs a Consent to Treatment form BEFORE the initial appointment is scheduled.
The Prospective Patient form can be accessed at the below link. Our Intake Specialist will be notified electronically of your request, and reach out to you within 2 business days.
Once we receive the Prospective Patient Form, we will call you to place the $200 deposit, schedule the appointment and answer any questions you may have.
Please Note: If you should need to reschedule or cancel the appointment, we require at least 2 business days' notice, otherwise the deposit will be forfeited.
* If the appointment is for a child of divorced or separated guardians, it is required that each guardian submits a Consent to Treatment form BEFORE scheduling the initial appointment. Detailed instructions for completing this form will be provided via email.
Prior to your appointment, you will need to complete the necessary paperwork. These forms will be sent via email to complete electronically. If not completed upon arrival, the forms will need to be filled out before the session can begin, potentially reducing time with your clinician. Your prompt completion is appreciated to ensure uninterrupted care during your session.
